Hospitals are full and so are emergency rooms.  There are not enough beds or providers to keep on doing it the old way.   There is also no budget to sustain it.  For hospitals to survive they need to care for just those patients who really need to be there.    

Hospitals are the most expensive place of care. At-home treatment not only benefits providers and patients but also payors.  However, in most plans the pharmacy and medical benefit are functionally separated and have their own budget.    Avoidance of inpatient care with the help of a prescription pharmaceutical is rare.  There is no standard way to resolve the conflict of interest between the pharmacy and medical benefit.  Currently, the most important reason that innovation is only very slow to happen is the lack of pharmacy benefit coverage for heart failure patients at imminent risk for hospitalization.   We all need to work together to resolve.
